m-Health systems can support patients with chronic conditions, such as Chronic Obstructive Pulmonary Disease (COPD) and Congestive Heart Failure (CHF), and help the healthcare professionals caring for them. Mobile devices such as tablet computers, and compatible medical sensors, can provide a platform for self-monitoring and self-managing these conditions, allowing patients to manage their symptoms better, improve their quality of life, and reduce unplanned hospital admissions. We present a modular architecture that was successfully tested during two field studies with 25 COPD and 34 CHF patients and is currently deployed in a randomized control trial involving 165 COPD patients.
